AUTHORIZATION
I certify that my answers are true and complete to the best of my knowledge. I authorize you to make such
investigations and inquiries of my personal, employment, educational and other related matters as may be necessary
for an employment decision. I hereby release employers, schools or individuals from all liability when responding to
inquiries in connection with my application.
In the event I am employed, I understand that false or
misleading information given in my application or interview(s) may result in discharge.
I understand that pursuant to the company's job application
processes I may be required to undergo drug testing. I further understand that if I refuse to take or fail the
drug test, I am disqualified from further employment consideration. I hereby knowingly voluntarily consent to the
company's request to undergo a drug test. I further release Rainsville Technology, Inc. and its officers, agents,
representatives and employees from any and all claims and liability for damages associated with or arising from my
submission to the test.
In consideration of my employment, I agree to conform to company
rules, regulations and policies, and agree that my employment and compensation can be terminated with or without cause,
and with or without notice at any time, at the option of either the company or myself. I understand that I maybe
required to work unscheduled work hours or shifts. I further understand that no supervisor, manager, officer or
representative of the company or any other entity, other than the president of Rainsville Technology, Inc. has any
authority to enter into any agreement for any specified period of time or to make an agreement for any specified
period of time or to make an agreement contrary to the foregoing.
U.S. law requires companies to employ only individuals who may legally work
in the United States - either U.S. citizens, or foreign citizens who have the necessary authorization. E-Verify compares information
from an employee's Employment Eligibility Verification, to date from U.S. Department of Homeland Security and Social Security Administration
